Managers, supervisors, and engineers are less likely to spend time in the field where they may observe housekeeping and plant condition issues. Their presence in and around the control room may also irritate or intimidate the operators, andor make them less likely to act quickly, decisively, and autonomously.

It had been necessary to de-pressurize and lock-out the reactors for repairs to the head of the leaking reactor. Apparently, the system actuating the retainer on the last reactor was inadvertently and unknowingly activated during maintenance work on the adjacent reactor. The valves activating the hydraulics for opening and closing the head retainers were not close to, nor in line of sight from the other reactors. There were no serious injuries but the cost of the incident, not including business interruption, was in the range of $250,000 to $1,000,000.

The following case history illustrates the consequences of inadequate internal sharing: During a routine start-up following a maintenance turnaround, the cover of the last reactor in a series abrubtly disengaged as the vessel pressure approached its normal operating pressure of 2,500 psig. The resulting depressurization projected the shell of the reactor downward about 4 feet, into the concrete floor, while launching the reactor cover and internals up and out into open areas. A vapor cloud of hydrogen and ammonia was released, which ignited outside the reactor.
